I understand that my esthetician will explain the procedure, benefits, potential risks, and expected outcomes. I understand that results vary based on individual skin conditions and adherence to aftercare.
I agree to disclose any medical conditions, allergies, medications, or skin sensitivities that may affect my treatment outcome, including:
I acknowledge that withholding information may increase my risk of reaction and I am responsible for any consequences related to undisclosed conditions.
I understand that skincare and advanced treatments carry inherent risks, including but not limited to: temporary redness, irritation, dryness, hyperpigmentation, sensitivity, allergic reactions, and other potential responses depending on the treatment.
I release Something Beautiful and its esthetician(s) from any liability for injury, reaction, or complications that may occur before, during, or after my treatment, except in cases of proven negligence.
I acknowledge that skincare outcomes depend on multiple factors including lifestyle, home care, skin condition, and consistency.
No specific results are guaranteed.
All services, treatments, and deposits are non-refundable.
If dissatisfaction occurs, adjustments or alternative solutions may be offered at the discretion of Something Beautiful, but monetary refunds are not provided.

A minimum of 20 hours’ notice is required to cancel or reschedule an appointment.
A $50 deposit is required before any service to secure my appointment.
I grant permission for before-and-after photos of my skin to be taken for professional documentation.
I understand images will never be posted publicly without my separate written consent.
